Midodrine

Midodrine, brand name Orvaten, is a selective alpha-adrenergic agonist that acts primarily on the alpha1-receptors in the smooth muscle of blood vessels. It works to constrict blood vessels, consequently increasing blood pressure. Its primary therapeutic use lies in the treatment of neurogenic orthostatic hypotension, a drop in blood pressure that occurs upon standing for patients with neurological disorders impacting autonomic function. Its effects on blood pressure can facilitate increased blood flow to the brain by preventing cerebral hypoperfusion. While Midodrine can effectively alleviate neurogenic orthostatic symptoms, it may cause side effects that include flushing, headaches, and nausea. Prescribers should weigh benefits and risks for each individual patient, always considering other treatment options. Safety in pregnant or breastfeeding women, or in children and adolescents, has not yet been established. Midodrine hydrochloride is available for oral use in 10 milligram (mg) strengths. It is essential that the patient follows the individualized dosing regimen developed by their healthcare provider, as Midodrine may cause rebound hypotension if stopped too quickly. Patients should be careful to rise slowly from a resting or lying down position to minimize feelings of dizziness or light-headedness, even while taking the drug.
